First Visit

On your 1st appointment, we will take a thorough history, and perform a focused exam to best assess any areas of complaint and/or determine how we can help your body function to the highest level possible.

Please take a few moments to complete to online paperwork below. If there is an aspect of the paperwork that is unclear, or doesn’t apply, please leave it blank and you can discuss anything further at your 1st appointment with the doctor.

Initial Evaluations (& treatment if deemed appropriate) are typically about 45 minutes. Please plan accordingly.

We do not perform imaging (X-Ray, MRI, etc) in-house. We will refer you to a local radiology clinic if deemed necessary in order to provide an accurate diagnosis and provide a higher level of care.

We are thrilled to provide Surface EMG Testing (sEMG) to our new patients. This test is completely safe and non-invasive. It is very similar to a cardiac EKG. The test allows us to obtain a baseline measurement of spinal stress, and objectively measure progress to assess how you are responding. This technology is cutting edge and we are proud to offer this assessment tool.
